Actually managed to play with a group for a bit today...
1. Where you are located
England, UK (my guildies are in the US, so I am too)
2. ISP you are using
PlusNet
3. Server you're on
Cimmeria
4. What you were doing at the time of the spike
Absolutely nothing. Just standing there, waiting for 19:31 BST (British Summer Time, GMT+1) to see if the lag spike happened. I was in the Acheronian Ruins. Normal latency registers at around 100ms.
5. Estimate of the time it happened at
19:31 BST
6. How long does it last
3-5 seconds. This is pretty much liveable, but I imagine in PvP situations it's not so great.

Additional Report
3. Server you're on
Cimmeria
4. What you were doing at the time of the spike
Returned to City of Tortage, entered the tavern.
Ping almost immediately shot to 3,600ms at the low points and close to 9,000ms at the high points.
5. Estimate of the time it happened at
20:15 BST?
6. How long does it last
Indefinitely. Switching instances failed to resolve, too.

Connect to the router through the onboard port on the motherboard.
That's about all I've got. Nobody make fun of my system
